Chronic Condition Management at Hawkins Medical Clinic

From Tuesday 1 July 2025, a new model of care planning called GP Chronic Condition Management Plans (GPCCMPs) will replace the current GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).

This is part of a national update designed to make chronic condition care more streamlined, personalised and easier to access.

What’s Changing?

  • A single, streamlined care plan will replace the previous multi-step system.
  • You no longer need to see multiple providers to get started — your GP and practice nurse can manage the plan together.
  • Care plans can be reviewed every 3 months to better support ongoing health needs.
  • You’ll still be eligible for up to 5 subsidised allied health visits per calendar year.
  • To prepare for future enhancements, we recommend patients register with MyMedicare and nominate Hawkins Medical Clinic as their regular practice.

What to Expect at Your Appointment

  • Your first appointment will be with a specially trained nurse, who will help develop a draft plan and discuss goals for managing your condition.
  • This is followed by an appointment with your GP, who will finalise the plan, provide education and make any necessary referrals.
  • You’ll receive a copy of your care plan, which can be shared with other health professionals involved in your care.

Who Is Eligible?

Patients with a chronic medical condition may be eligible. Your GP will assess your needs and determine whether a GPCCMP is right for you.
